An Accredited Exercise Physiologist (AEP) is a university-qualified allied health professional registered with Exercise & Sports Science Australia (ESSA), who specialises in the prevention, treatment, and management of acute and chronic health conditions through evidence-based exercise interventions.

At JY Exercise Physiology, we design safe, personalised exercise programs tailored to your individual needs, medical history, and goals, helping you improve physical and mental wellbeing, regain functional capacity, and enhance your overall quality of life.

Unlike personal trainers, Accredited Exercise Physiologists have extensive clinical training in physiology, anatomy, biomechanics, and evidence-based practice, and are recognised healthcare providers within the Australian healthcare system.

We accept clients under the following funding schemes:

Private Health Insurance – We accept all major Australian private health funds under extras and allied health cover. We recommend contacting your fund to confirm your exercise physiology entitlements before your appointment.

Medicare – Chronic Disease Management (CDM/EPC) Eligible patients can access subsidised exercise physiology sessions through a GP Management Plan and Team Care Arrangement. Ask your GP for a referral to an Accredited Exercise Physiologist under the Chronic Disease Management (CDM) program. Medicare currently subsidises up to 5 allied health sessions per calendar year.

NDIS – National Disability Insurance Scheme We support NDIS participants with tailored exercise programs under Capacity Building – Improved Daily Living or Improved Health and Wellbeing funding. We work with self-managed, plan-managed, and agency-managed participants.

WorkCover QLD – We provide evidence-based injury rehabilitation services for clients with accepted WorkCover QLD claims. A referral from your treating GP or specialist is required to get started.

DVA – Department of Veterans’ Affairs We proudly support Australian veterans holding DVA Gold and White Cards, providing fully funded exercise physiology services. A referral from your GP or treating specialist is required.

Other Insurances – We also accept referrals through other third-party insurers and compensation schemes. Please contact us to discuss your specific situation.

Private Clients – No referral is required for private clients. You can book directly with us at any time.

Consultation lengths at JY Exercise Physiology are as follows:

Initial Consultation: Approximately 60 minutes

Subsequent Session (Standard): 30 minutes

Subsequent Session (Extended): 60 minutes

The length of your ongoing sessions will be discussed and recommended based on your individual needs, condition, and goals during your initial consultation.
Your initial consultation at JY Exercise Physiology is a comprehensive, one-on-one assessment designed to give us a thorough understanding of your health, medical history, and goals. Here is what to expect:

Comprehensive Health Assessment – We will review your medical history, current health concerns, medications, previous injuries, and any relevant specialist or GP reports. If you have a referral letter or medical reports, please bring these to your first appointment.

Physical Assessment – Depending on your condition and goals, we may assess your movement, strength, balance, flexibility, cardiovascular fitness, or functional capacity.

Goal Setting – We will discuss your short and long-term health and movement goals to ensure your program is aligned with what matters most to you.

Personalised Exercise Program Design – Based on your assessment, we will begin designing a tailored, evidence-based exercise program specific to your needs, abilities, and goals.

Education & Next Steps – We will explain your program, answer any questions, and discuss the recommended frequency and duration of follow-up sessions.

Please wear comfortable, active clothing and supportive footwear to your appointment. Arrive a few minutes early to complete any required intake paperwork.
No referral is required for private clients. You can contact JY Exercise Physiology directly and book an appointment at any time.

However, a referral is required to access subsidised services through the following funding schemes:

Medicare (CDM/EPC): Referral required from your GP under a Chronic Disease Management plan

WorkCover QLD: Referral required from your treating GP or specialist

DVA: Referral required from your GP or treating specialist

NDIS: No formal referral required, but you will need access to Capacity Building – Improved Daily Living or Improved Health and Wellbeing funding in your NDIS plan

If you have been referred by your GP, specialist, surgeon, or another allied health professional, simply contact us with your referral and we will take care of the rest.

Pre-surgical Exercise Physiology (Prehabilitation) Seeing an Exercise Physiologist before surgery known as prehabilitation or “prehab” can significantly improve your surgical outcomes. Pre-surgical exercise programs help to build strength, improve cardiovascular fitness, and optimize your physical condition before going into the operating room. Research consistently shows that patients who complete prehabilitation recover faster, experience fewer post-surgical complications, and return to function sooner.

Post-surgical Exercise Physiology (Rehabilitation) Following surgery, an Accredited Exercise Physiologist will work alongside your surgeon and treating team to guide your recovery through a structured, progressive exercise rehabilitation program. Post-surgical exercise physiology is commonly used for joint replacements (hip and knee), spinal surgery, cardiac surgery, and oncology recovery.

Medicare (CDM/EPC): A Medicare rebate applies per session for eligible patients. Your gap payment will be confirmed at the time of booking. Medicare rebate per session $61.80.

Private Health Insurance: Rebate amounts vary depending on your fund and level of cover. We recommend checking with your insurer prior to your appointment

● NDIS, WorkCover QLD & DVA: Fees are billed directly to your funding scheme, no out-of-pocket cost for eligible clients

We understand that life can be unpredictable. However, to ensure fairness to all clients and to maintain appointment availability, the following policy applies:

Late Cancellation & Reschedule We kindly ask that you provide a minimum of 24 hours notice if you need to cancel or reschedule your appointment. Cancellations and reschedules made with less than 24 hours notice may incur a late cancellation fee of $35.00.

No Show Failure to attend a scheduled appointment without prior notice will be considered a no-show and may incur a full session fee.

How to Cancel or Reschedule Please contact us as early as possible by phone or email to cancel or reschedule your appointment.

Please note: Late cancellation and no-show fees cannot be billed to Medicare, NDIS, DVA, or WorkCover QLD and will be the client’s responsibility.

Home Visits & Mobile Services

Yes. JY Exercise Physiology proudly offers mobile exercise physiology home visit services across Brisbane South, bringing professional, accredited exercise physiology care directly to you.

Home visits are ideal for clients who:

● Have difficulty travelling due to injury, disability, or health conditions

● Are NDIS participants receiving support in a home or community setting

● Are DVA clients requiring in-home allied health services

● Are older adults or seniors who prefer the comfort and familiarity of their home environment

● Are post-surgical clients in the early stages of recovery

We service home visit clients across Upper Mount Gravatt, Sunnybank, Macgregor, Wishart, Runcorn, Eight Mile Plains, Calamvale, Nathan, Robertson, Holland Park, and surrounding Brisbane South suburbs.

Yes, absolutely. Many of our clients alternate between home visits and clinic sessions depending on their needs, schedule, and stage of rehabilitation. We are flexible and will work with you to find the arrangement that best supports your health journey and goals.
Home visit fees may differ from standard clinic consultation fees. Additional travel costs may apply depending on your location. Please contact JY Exercise Physiology directly to discuss home visit pricing and whether your funding scheme covers mobile services in your area.

NDIS Support

Yes. JY Exercise Physiology is an NDIS provider supporting participants with tailored, evidence-based exercise programs under Capacity Building – Improved Daily Living or Improved Health and Wellbeing funding.

We support NDIS participants with:

● Chronic disease management, health condition support and disability

● Mobility, strength, and functional independence programs

● Falls prevention and balance training

● Post-injury or post-surgical rehabilitation

● Mental health and wellbeing through exercise

● Home visit and community-based sessions

How to get started with NDIS:

1. Confirm you have Capacity Building – Improved Daily Living or or Improved Health and Wellbeing funding in your current NDIS plan

3. Advise whether you are self-managed or plan-managed

4. We will confirm eligibility, discuss your goals, and book your initial consultation

5. If you are plan-managed, provide your plan manager’s details and we will handle the billing directly If you are unsure whether exercise physiology is covered under your NDIS plan, contact your NDIS planner, support coordinator, or plan manager or reach out to us and we will help guide you through the process.

WorkCover QLD

Yes. JY Exercise Physiology provides evidence-based injury rehabilitation for clients with accepted WorkCover QLD claims, helping you recover safely, rebuild strength, and return to work.

We assist WorkCover QLD clients with:

● Work-related musculoskeletal injury rehabilitation

● Chronic pain management following a workplace injury

● Functional capacity building and return-to-work programs

● Post-surgical rehabilitation following a work-related injury

How to get started with WorkCover QLD:

1. Ensure your WorkCover QLD claim has been accepted

2. Obtain a referral from your treating GP or specialist for exercise physiology treatment

3. Contact JY Exercise Physiology with your claim number and referral (work capacity certificate)

4. We will liaise with your WorkCover case manager and treating team

5. Book your initial consultation, fees are billed directly to WorkCover QLD

Medicare & CDM Plans

Yes. JY Exercise Physiology accepts Medicare referrals under the Chronic Disease Management (CDM) program, also known as an Enhanced Primary Care (EPC) plan.

Medicare subsidises up to 5 allied health sessions per calendar year for eligible patients with a chronic or terminal medical condition managed by their GP.

Conditions commonly referred under Medicare CDM include: Diabetes, cardiovascular disease, osteoporosis, osteoarthritis, obesity, chronic respiratory conditions, cancer, and other chronic health conditions.

How to get started with Medicare:

1. Visit your GP and ask about a Chronic Disease Management (CDM) plan and Team Care Arrangement (TCA) with a referral to an Accredited Exercise Physiologist

2. Obtain your GP referral letter and Medicare paperwork

3. Contact JY Exercise Physiology with your referral

4. We will confirm your Medicare eligibility and advise on any gap fees prior to your appointment

5. Book your initial consultation

Please note: There will be a gap fee depending on the service fee. Medicare will rebate $61.80 per session

DVA Veterans Support

Yes. JY Exercise Physiology proudly supports Australian veterans by accepting DVA Gold Card and DVA White Card clients, providing fully funded exercise physiology services with no out-of-pocket costs for eligible veterans.

We assist DVA clients with:

● Chronic pain management and musculoskeletal rehabilitation

● Cardiovascular health and fitness programs

● Mental health and wellbeing support through exercise
