But chronic disease management is about more than just moving, it’s about knowing how to move safely and effectively. What Is an Accredited Exercise Physiologist ?
An Accredited Exercise Physiologist is a university-trained allied health professional who designs and delivers exercise programs specifically for people with chronic, complex, or high-risk health conditions. Unlike a personal trainer, an AEP is trained to work safely with medical complexity, cardiovascular disease, diabetes, cancer, chronic pain, neurological conditions, and post-surgical recovery. Exercise physiology adapt programs in real time as your health changes.
Every program at JY Exercise Physiology starts with a thorough assessment of your medical history, current function, and personal goals, so the exercise you’re doing is not generic, it’s prescribed specifically for you.
Why Exercise Matters So Much for Chronic Disease
Exercise is one of the most powerful, low-cost, side-effect-free tools we have for managing chronic disease. Structured, supervised exercise can improve cardiovascular health, support blood glucose control, reduce pain and improve joint function, build strength and balance to lower fall risk, ease anxiety and depression, boost energy, and reduce the risk of complications like further cardiac events or hospital readmission.
The key word is structured. Random or poorly-dosed exercise can do little or even cause harm for someone managing a complex condition. Exercise prescribed and monitored by an AEP is dosed like a medication: the right type, intensity, and progression for your specific condition and stage of recovery.

- Cardiovascular disease (including post-heart attack rehab and hypertension)

How the Medicare Rebate Works
One of the most common questions we’re asked is whether exercise physiology is covered by Medicare and the good news is, for many people managing a chronic condition, it is.
The GP Chronic Condition Management Plan (formerly known as EPC/CDM)
If you have a chronic condition, your GP can create a GP Chronic Condition Management Plan (GPCCMP) the simplified plan that replaced the older Enhanced Primary Care (EPC) and Chronic Disease Management (CDM) referral system.
Under this plan:
- You can be referred for up to five allied health visits per calendar year, subsidised by Medicare
- These five visits are shared across all allied health providers you see for example, you might use two visits for exercise physiology and three for podiatry, or all five for exercise physiology if that’s your primary need
- As of July 2026, the Medicare rebate is $63.40 per eligible session
- A gap fee typically applies, since the rebate covers part not all of the consultation cost
- Your GP will need to issue a new referral each calendar year, as these plans don’t automatically roll over
How to Get Started
- Book an appointment with your GP and ask whether you’re eligible for a GP Chronic Condition Management Plan
- Ask your GP to include exercise physiology in your referral, and to name JY Exercise Physiology as your provider (include email, phone number and address).
- Bring your referral to your first appointment with us or your GP will email us a copy.
- We’ll guide you through the rest, including how your rebate is processed
If you’re not eligible for a Medicare-subsidised plan, private health insurance may also cover exercise physiology under “extras” cover, it’s worth checking your policy, as coverage varies by fund and level of cover.
